Service Request Form & Website Widget

Use the Service Request Form to capture new opportunities from your website. Submissions flow into Leads.

Configure the Form

1. Go to Settings > Service Request Form.
2. Set your public form URL slug.
3. Add service types customers can choose from.
4. Add your welcome message.
5. Save changes.

Share the Form

1. Copy your public form link from settings.
2. Paste it on your website, social page, or email signature.

Embed on Your Website

1. In Settings > Service Request Form, copy the embed code.
2. Paste it into your website page where you want the form to appear.
3. Publish your website changes.

What Happens After Submit

1. A lead is created automatically in Leads with source Web Form.
2. Your team can update stage, add notes/tasks, and convert to job or estimate.
